Patient Advantage services will impact the healthcare needs of your staff and their families no matter the size of your organization.
Services can be customized to fit your organization's scheduling requirements and budget. The program can assist staff with:
- Managing medical savings accounts when faced with a chronic disease
- Working more effectively with current health coverage or plans with defined benefit options
- Addressing care options for those employees that may not be covered by a comprehensive benefit plan
Here is How Patient Advantage Works for You
The following services are provided through Patient Advantage:
1. The Information Search
- A request for an Information Search can be requested for an employee or a family member who has a serious Illness. A professional researcher will search the Internet and available medical publications for information regarding the member's diagnosis, questions and treatment options.
- The information is formatted into a report and edited to a ninth grade reading level. The report is hand delivered by a registered nurse who can explain the material and answer any questions.
- Reports are also available for the treating physician.
2. The Nurse Advocate
Nurse Advocates are available to help employees and their families navigate the healthcare system. The services can include:
- Help communicating with the doctor
- Getting answers to general health questions or learning more about the care of specific medical conditions
- Coordinating of a complex plan of care
3. The Proactive Nurse Model
Business models vary, as do the needs of your staff. The Proactive Nurse Model from Patient Advantage looks at multiple key elements to help you manage the healthcare risk of your organization.
- An initial focused health screening is done for all of the covered individuals
- Insurance Claims Data (when available) is reviewed to identify utilization patterns and other healthcare risks
- Options are presented to minimize risks in key areas such as wellness promotion, injury prevention, health education and advocacy support for chronic disease
- Quarterly data is provided so you can effectively track trends, monitor intervention effectiveness and assess the impact of the Patient Advantage program
